If you enroll to receive Electronic Statements, you will no longer receive your paper statements via mail.

To begin receiving your statements electronically:

• You must have Adobe Reader version 5.0 or higher and Internet Explorer 5.5 or later installed on your computer. A current version of Adobe Reader can be downloaded at no charge at www.adobe.com.
• Please read the following agreement and disclosures. If you agree, click 'continue' to view the next page for the online Enrollment Form.
• Complete the enrollment application with the appropriate account information (loan and savings accounts are ineligible for this service) and click 'Submit Form.'

Agreement and Disclosures

I understand that if I receive my monthly or quarterly checking or savings account statement via e-mail, I will not receive a monthly paper statement.

I understand and agree to submit my e-mail address to Bartow County Bank as a part of this application.

I understand that if I change my e-mail address, I shall notify Bartow County Bank in writing or via e-mail of my new e-mail address. If I fail to notify Bartow County Bank of my new e-mail address, I understand that I may not receive my monthly or quarterly statement. I also understand that the bank may assess a fee for the re-transmission of my statement to the correct e-mail address.

I agree that I have the necessary software installed to receive my statement electronically.

I understand that my PIN (Personal Identification Number) will be four to eight digits and will be the PIN I will use on this application. I also agree to notify Bartow County Bank if I decide to change my PIN. I understand that Bartow County Bank may assess a fee for PIN re-issuance.

I understand that the security of my PIN is my responsibility and that anyone who has access to my PIN will be able to view my e-mail statement. I further understand and agree to notify Bartow County Bank immediately if my PIN should become compromised or known to individuals who are not authorized by me.

Only the statement of the account number(s) listed in this application will be e-mailed to the address listed in this application. The statements of other Bartow County Bank customers cannot be mailed to the same e-mail address listed in this application. Each customer statement must be e-mailed to a unique e-mail address.

I understand that Bartow County Bank reserves the right to amend this disclosure (agreement) in any manner and at any time. If Bartow County Bank does so, the bank will give me at least ten (10) days advance written notice. The bank also reserves the right to terminate this agreement by sending a notice of such termination by first-class mail to my last known address on the bank's records.

I understand that I may withdraw my consent to receive my statement electronically by written notice to Bartow County Bank , PO Box 1030, Cartersville, GA 30120 or send an e-mail message to the bank at statements@bartowcountybank.com. I understand that the bank must receive the notice at least ten (10) days before the statement is to be e-mailed to my current e-mail address.
